This Privacy Policy discloses the policies of www.wellneuro.com (the “Website”) and its operator Wellness Institute of Neurodevelopment, LLC, CBF Pediatric Services LLC, CBF International, PSC (collectively “we” or “us”) regarding the collection, use and disclosure of information you submit to us through the Website.

This privacy statement applies to information collected through any websites, mobile apps or online or mobile-enabled technology or digital tools, social media  that display or link to this privacy statement (collectively, our “Online Platforms”). This privacy statement also applies to information shared with us by our approved partners and other third-party sources. 

This privacy statement does not apply to (i) information collected from you offline or through websites and other online services that do not display or link to this privacy statement, including third party websites to which our online services may link, or (ii) information that would be considered “Protected Health Information” under HIPAA. Wellness Institute of Neurodevelopment use and disclosure of patient Protected Health Information is set forth in our Notice of Privacy Practices. 

User Consent 

By accessing or otherwise using the Website, you agree to the terms and conditions of this Privacy Policy, which is incorporated into and forms a part of the Website’s terms of service.  You expressly consent to the processing and use of your information as described herein.Information collection and how it is used.  

General Information 

You do not have to give us any personal information, such as your name, e-mail address or Social Security number, to visit the product information portion of our website. 

The sites collect personal information when you sign up for a trial or paid subscription to the WIN Membership Program or store. When you sign up, we ask for your name, email address, phone number, demographic information, and financial information such as credit card information. 

We also collect data that is associated with the foregoing, as well as "Anonymous Information," which is information that is not associated with or linked to your Personal Information and does not, based on our understanding, permit the identification of individual persons. 

When you sign up for a paid subscription, we will ask you to enter your credit card or ACH information. This information is encrypted on computer systems that are secured in a locked cage at a data center co-location facility rented from a third party by the Wellness Institute of Neurodevelopment (WIN). 

WIN automatically receives and records information on the server logs of the third party from their browser, including your IP address and the page you request. 

WIN uses information for the following general purposes: products and services provision, billing, identification and authentication, services improvement, contact, research, and anonymous reporting. 

WIN will send you strictly service-related announcements on rare occasions when it is necessary to do so. For instance, if our Service is temporarily suspended for maintenance, we might send you an email. Generally, you may not opt-out of these communications, which are not promotional in nature. If you do not wish to receive them, you have the option to deactivate your account. 

If you wish to subscribe to our blog, newsletter, or membership programs, we will use your name and email address to send information to you. Out of respect for your privacy, we provide you a way to unsubscribe by clicking on the link within the email. 

We use other third parties such as a chat service provider to provide customer service to you, and a credit card processing company to bill you for goods and services, an email service provider to send out emails on our behalf. When you sign up for these services, we will specify what personally identifiable information is being shared as necessary for the third party to provide that service. 

If you are a patient and you sign up for Kareo Patient Portal, WIN Membership Program and elect to use the online patient payment service, your transaction will be processed by Stripe, Inc., and the information you provide to Stripe is subject to its privacy policy.  

If you elect to sign up for the WIN Store and use the online payment service, your transaction will be processed by Nutridyn, Inc. Your credit card and billing information, in addition to the online payment service and your transaction will be processed by Nutridyn, Inc. and the information you provide to Nutridyn Inc. is subject to its privacy policy. Please, review their privacy policy for additional information.  

Collection of Personal Information 

We collect Personal Information you voluntarily submit to us through the Website.   It is optional for you to utilize this Website.  If you elect to utilize certain aspects of this Website, we may ask you to provide us with certain personal information about yourself, such as your first and last name, mailing address (including zip code), company name, email address and telephone number.  When you order products or services, we may also ask you to provide us with your credit card number, expiration date and authentication codes or related information.  If you do not provide certain mandatory information for a particular activity that requires it, you will not be permitted to engage in that activity.  We may offer contests, sweepstakes or other contests that will require you to sign up and provide additional Personal Information.  In that case, we will collect the information provided along with your involvement in such activity.  Additionally, we reserve the right to collect any other information that you voluntarily enter, including Personal Information, into any postings, comments, feedback, or forums on the Website, social media sites and others. 

We may, but are in no way obligated to, provide for referral, or tell other programs that allow you to recommend the site or an article to a friend.  If you choose to do so, we will ask for your referral’s name and email address. We will automatically send your friend one or more communications with either the product you have purchased or an invitation for him or her to visit the Website.   The communication will allow the referral to unsubscribe from further communications.  We will collect this information for the sole purpose of sending an invitation to visit the site, sending their product, and tracking the success of any referral program.    We will also collect your e-mail address and the full content of your e-mail, including attached files, and other information you provide.  We may use and display your full name and email address to your referral.  Additionally, we use your email address to contact you about the status of your referral.

IP Addresses are automatically reported by your browser each time you view a web page or use our Service. 

IP addresses may be used for various purposes, including:

3rd Party Tracking 

The use of cookies by any tracking utility company is not covered by our privacy policy. We do not have access or control over these cookies. Tracking utility company may use session ID cookies and/or persistent cookies. 

We use Local Storage, such as HTML5, to store content information and preferences. Third parties with whom we partner to provide certain features on our website or to display advertising based upon your Web browsing activity also use HTML5 to collect and store information. Various browsers may offer their own management tools for removing HTML5. 

Behavioral Targeting / Re-Targeting 

We partner with a third-party ad network to either display advertising on our website or to manage our advertising on other sites. Our ad network partner uses cookies and Web beacons to collect information about your activities on this and other websites to provide you targeted advertising based upon your interests. If you wish to not have this information used for the purpose of serving you targeted ads, you may opt-out by clicking here (add link). Please note this does not opt you out of being served advertising. You will continue to receive generic ads. 

Protected Health Information 

If you are a patient using the website, please do not provide WIN website with Protected Health Information (PHI). If you are a patient use Kareo Clinical, Kareo Billing or Kareo Marketing (including Kareo DoctorBase), the information you provide to us through those Services may be considered PHI and will be protected by Kareo as required by federal and state laws. 

Information collection, sharing and disclosure. 

WIN may be required to disclose personally identifiable information or protected health information under special circumstances, such as to comply with subpoenas or when your actions violate the WIN Terms of Service. 

WIN may share demographic information with business partners, such as "7% of WIN customers are in the Los Angeles region" or the like. No personally identifiable information or protected health information will ever be used without your permission (i.e., we will ask you before using a quote or testimonial). 

Third party vendors, including Google, show our ads on sites on the Internet. Third party vendors, including Google, use cookies to serve ads based on a user's prior visits to our website. Users may opt out of Google's use of cookies by visiting the Google advertising opt-out page. 

WIN does not rent, sell, or share personal information about you with other people or nonaffiliated companies for promotional purposes except to provide products or services you've requested or when we have your permission. 

With your consent, we do share your name and email with certain partners we may work with. If you would not like your information shared with these partners, uncheck the box when asked this option or notify us via info@wellneuro.com. 

It may be necessary to share information in order to investigate, prevent, or take action regarding illegal activities, suspected fraud, situations involving potential threats to the physical safety of any person, violations of WIN's terms of use, or as otherwise required by law. 

We will transfer information about you if WIN is acquired by or merged with another company. In this event, WIN will notify you by email or by putting a prominent notice on the WIN website before information about you is transferred and becomes subject to a different privacy policy. 

We use technology to passively collect and store certain anonymous information that is not tied to a specific identified user which may include, but is not limited to your Internet protocol address, browser type and operating system.   We, or our authorized third-party representatives may use Cookies and navigational data like Uniform Resource Locators (URL) to gather information regarding the date and time of your visit, the site you immediately came from and the site you immediately go to after accessing the Website.  It will also collect data about the use of the Website including what programs, applications, advertisements, and terms you searched, viewed or clicked.  Should you be directed to any other site from the Website, we are not responsible for the acts of other sites and this Privacy Policy does not apply to that site. 

We may engage in remarketing to market our sites across the web. When a user visits our site, a cookie is dropped on the user. Users with this cookie may be targeted across advertising networks to receive relevant advertisements.

Collection and Use of 3rd Party Personal Information 

You may also provide personal information about other people, such as their name, email address and phone number. It is your responsibility to get permission from anyone whose personal information you provide to us. We will only use that personal information for the purpose of completing your request. 

Social Media Widgets 

Our website includes Social Media Features, such as the Facebook Like button and Widgets, such as the Share this button or interactive mini-programs that run on our website. These Features may collect your IP address, which page you are visiting on our website, and may set a cookie to enable the Feature to function properly. Social Media Features and Widgets are either hosted by a third party or hosted directly on our website.
